Question on packet loss in dashboard

Still a relative newbie to Meraki.   How is packet loss calculated by Meraki?  In mine, I am seeing every now and then  access points showing 90+% packet loss.  I'm having a hard time believing that.  How is it calculated and if it's really that high, what can cause something like that?
