I have had seen this issue too.  Interestingly,  I've only seen this when using Cisco ISE as the Radius server.  I don't see this when Microsoft NPS is acting as the radius server.  I see good traffic through the Firewall so wondering if this is a "timing" issue.  Going to do some packet captures when I have time to see if I can get more info into the problem.  ...
