I asked via my ticket, but I got a pretty general answer. Not a direct quote: Engineering implemented some back-end changes which inadvertently irritated SAML. The changes were backed out, which resolved the problem.

From memory, this happens when the SAML roles are not correctly mapped to Meraki roles. Go to Organization/Administrators/SAML Login History. Look for an error there.
